Consent to Treat Form
Consent to Treat forms are sent out to players' families at the time of registration. Coaches and managers can view Consent to Treat forms in each player's profile on Crossbar under their Team Roster.
USA Hockey Injury Reporting
When an injury occurs on the ice during a Nashoba Youth Hockey sanctioned event (ie. practice, Skills, game, or other NYHA event), please:
- Notify Health & Safety (see contacts below) as soon as possible
- If a player, coach, or volunteer visits a doctor, please complete the USA Hockey Injury Report Form
- When possible, provide us with any additional information (ie. video footage)
